Medical Questionnaire (MD 911A OR 911E)

The Medical Questionnaire, form MD911A OR 911E, is an initial medical screening tool to allow us to assess your potential medical suitability for employment in the NZDF. It will be sent to you electronically for you to complete and send back to us. If you list any medical issues please provide as much detail as possible to allow our doctors to fully assess the situation. If there are any queries, our medical staff will write to you directly requesting more detailed information dependant on the issue. You need to be assessed as fit to proceed to full medical (FTPTFM) to continue with the selection process so it will be in your best interests to provide as much information as possible. You must declare any injury, ailment or condition that you have.

Medical Examination

You will need to undergo a full medical examination with your current military doctor and have the MD 911A OR 911E & MD 913 OR 918 Medical Forms (plus any extra forms as applicable for pilots and divers) completed. You will also need to complete a Multi-Stage Fitness Test (Beep Test) at the required level for your branch and have a PTI sign a record form. If Army, you will need to complete a Required Fitness Level test (RFL) which is very similar to the UK and Aus fitness tests. You will need to send the completed medical and fitness forms plus a copy of your full medical file to your local High Commission and the information will be sent directly to our medical team in NZ.

Please be aware that the military medical is totally separate to your immigration medical - we need a military doctor’s assessment of your health, as they understand the environmental issues of being in the Defence Force. Once our doctors have received all of your information they will assess your fitness for enlistment and will grant a medical clearance to enlist if everything is in order. If you incur a medical condition between your medical in the UK and your arrival in NZ please contact us as soon as possible so that we can manage the situation accordingly.

Please note that we do not need to see a copy of your immigration medical and we cannot accept this in lieu of a military medical. If you are not able to see a military doctor you will need to have the MD 911 Form completed by a civilian doctor. If this incurs a cost you may be able to claim a refund on successful enlistment into the NZDF so please keep the receipt.
